You can't still apply if you have a disability or long term condition that qualifies under the Equality Act, we have a form your GP or Health Professional can sign.
If you do not have one of Purpl’s accepted documents, you can ask your GP or another health professional to complete our Disability or Long-Term Condition Confirmation Form.
Please note: your doctor may charge you to complete the form so check you don’t have one of our accepted documents first.

Once you’ve filled in Section 1, email or take the form to your GP or health professional. They will complete Section 2. |
| Step 4 | Your GP/health professional emails the form to Purpl from an NHS email address. |
If they approve the form we will verify your account If they don't approve the form we will email you to let you know other options. |
We cannot accept completed forms directly from members unless the email contains the original email from the health professional.
Who can complete the form?
This may include a:
- GP
- Consultant
- Psychiatrist
- Specialist nurse
- Occupational therapist
- Other qualified health professional involved in your care
They must be able to confirm that your disability or long-term health condition has a substantial and lasting impact on your daily life.
Will my GP charge me?
Some GP surgeries or health professionals may charge for completing forms. Any fee is set by them and is not paid to Purpl.
What if my health professional will not complete it?
Check whether you have another accepted document or whether a different health professional involved in your care can complete the form.
You can also contact the Purpl support team and explain what evidence you have. We can tell you whether it may be suitable.
